Mission
The Community Mammography Access Project's mission is to institutionalize a reliable mobile screening mammography program for our member community health centers to increase access to mammography services and reduce late stage breast cancer diagnosis among medically underserved women in Santa Clara County through the collaborative effort of community based mobile mammography services and an existing safety net of healthcare providers.

Objective/Purposes
The primary objective and purposes of the Community Mammography Access Project shall be:

• To provide culturally and linguistically appropriate breast cancer education, screening and referral services.
• To work in collaboration with the network of community-based health centers, Santa Clara Valley Health and Hospital System, and existing safety net of healthcare providers.
• To develop and implement a standardized breast health delivery model for member clinics, building in a comprehensive patient tracking, reporting and follow up system.

CMAP Technical Assistance
In order to provide member clinics the support and guidance needed to navigate and coordinate CMAP events, the Women's Health Partnership has assigned a CMAP Coordinator for all technical assistance for CMAP activities.

The CMAP Coordinator will assist the CMAP Clinic Coordinator throughout the event planning (including the day of the event). Technical assistance include but not limited to:

• Event Planning Teleconferences
• Event promotion/Patient recruitment/Outreach
• CMAP support services (i.e. patient incentives, refreshments, travel vouchers)
• Clarification of CMAP operational procedures
• Site assessment, arrangement, and logistics planning
• Patient registration/scheduling
• Event implementation and event logistics
• Communication between Inner Images and VMC
• Voice for all concerns and issues of CMAP clinic staff at CMAP advisory meetings, Inner Images and VMC
• Assist with timely results from VMC and follow up info with referral patients
• Assist with tools and materials to create efficient data collection methods for ongoing program evaluation and quality assessment
