Background & Experience
Lori Osorio, MPH, is a Program Coordinator at the Community Health Partnership. She is responsible for planning, coordinating and facilitating the San Jose AIDS Education and Training Center program which offers education, training and capacity building projects specifically designed to improve HIV/AIDS care, treatment and prevention services for clinical care providers. She also assists with Community Health Partnership’s Health Home Transformation Initiative, a major project whose aim is to support community health centers achievement of Patient Centered Health Home (PCHH) transformation. As a trainer and practice coach, Lori supports clinics’ transformation by training leadership and direct-service staff on panel management, health coaching and practice redesign. She is currently utilizing these training and facilitation skills to help lead a comprehensive primary care workforce education and training program to support CHP’s members deliver high quality health care.
