Community and Population Health Programs
Community Health Partnership offers an array of programs that fall under four main categories in our Community and Population Health Division: Community Health, Population Health, Workforce Development, and Provider Training. Through these programs, we offer health services, education, technical assistance and training to our member health centers and greater community.
The overall goal of our Community and Population Health Division is to improve the health of individuals and communities served by our member community health center network in Santa Clara and San Mateo counties.
Community Health Partnership provides outreach and services directly to the community in the areas of women’s health.

Community Mammography Access Project
Community Health Partnership works with our member community health centers on quality improvement and research initiatives that are geared towards advancing systems of data collection, access, and quality of care that ultimately improve the health outcomes of their patient populations. We coordinate with our health centers’ clinicians to study the health of entire communities, so clinicians can best evaluate how patients are doing as a group.
Community Health Partnership is home to workforce development and provider training programs that are focused on delivering education and providing technical assistance to health care professionals.
