The Health Trust funds Diabetes Community Care Coordination Project (DCCCP)
The Community Diabetes Project is happy to announce that The Health Trust has approved funding for the implementation of the second year of the Diabetes Community Care Coordination Project (DCCCP). The main goal of the DCCCP is to provide uninsured and underserved persons who have diabetes with a medical home. The Community Diabetes Project has led this program and used previous funds from The Health Trust to provide case management services and to cover the cost of up to eight medical visits for approximately one year. Open House inaugurates the Partnerships new location
Community Health Partnership welcomed 150 guests to a festive Open House on December 14th, 2006, at its new location in Santa Clara. Attendees included clinic staff, Medical Directors and Executive Directors, community members, Ken Yeager from the Santa Clara County Board of Supervisors and a representative from Congresswoman Zoe Lofgrens office. Highlights of the evening included a dedication of the new conference room by Executive Director Rhonda McClinton-Brown to Kaiser Permanente, and an inauguration of the brand-new Community Health Partnership Health Education & Training Center, dedicated to the formerly independent HETC. Community Mammography Access Project (CMAP) blooms in 2007
Over a year ago, CMAP kicked-off the vision that had been in the making since 2001: to make mammography screening more accessible to low income, under and uninsured women in Santa Clara and southern San Mateo counties. The seeds are blooming in 2007as CMAP rotates among 17 member community health center sites where women will receive a clinical breast exam and screening mammogram (visit the CMAP website for details). A Counseling, Testing and Referral (CTR) program for communities of color and underserved high risk populations
This collaborative project between Asian Americans for Community Involvement (AACI) and the Partnership will work collectively with Santa Clara Countys current HIV testing program to effectively reach high risk people of color and to reduce new infections of HIV disease by expanding HIV Alternative Testing Sites in Santa Clara County. The program proposes to reach two primary target populations: Asian-Pacific Islanders and Latino men who have sex with men, and transgender persons of color. Secondary populations the program aims to reach include at-risk and high risk heterosexual men and women as they are identified through the primary focus populations. Click here to read more. |

Executive Director celebrates 10 years at the Partnership!
The Partnership honors Executive Director Rhonda McClinton-Brown for 10 years of dedicated service. Mrs. McClinton-Brown joined the Partnership in December of 1996 and has been instrumental to the success, growth, and impact of Community Health Partnership in the last decade. Staff, Board of Directors members, friends and family paid tribute to Mrs. McClinton-Brown at a surprise party held in late January. |

Senator Elaine Alquist names Rhonda McClinton-Brown San Jose Woman of the Year!
On February 23, 2007, California State Senator Elaine Alquist honored the Partnerships Executive Director, Rhonda McClinton-Brown, by designating her as San Jose Woman of the Year. The Senate District 13 City Woman of the Year Program recognizes the hard work and dedication of strong women leaders who make a difference in their communities. |

Ravenswood Family Health Center becomes Full Member
In November 2006, the Community Health Partnership Board of Directors voted to instate Ravenswood Family Health Center (RFHC) as a Full Member. Full Membership entitles RFHC to voting privileges and collaborative funding, as well as participation and inclusion in the Medical Directors and Clinic Managers peer groups, participation in various Partnership programs, and a forum to develop collaborative relationships with other safety net providers in the County. |

The Partnership has undertaken a quality improvement (QI) project in collaboration with member community health centers (CHCs). Funded by Kaiser Permanente, the project has afforded the opportunity to develop the infrastructure for supporting CHCs in quality improvement projects with the goal of delivering the highest quality patient care for our community. For the 2006-2007 fiscal year, the QI Committee (comprised of representatives from member clinics) decided to focus on hypertension. Click here to read more. |

The Partnership welcomes new Associate Member Pacific Free Clinic (PFC)
In November 2006, the Community Health Partnership Board of Directors voted to establish PFC as a new Associate Member. As such, PCF will participate in the Partnerships Medical Directors and Clinic Managers peer groups, join in the Partnerships various programs, and have access to developing collaborative relationships with other safety net providers in the County. The Health Trust and Council on Aging grants Womens Health Partnership $20,000 to revamp Healthy Women, Healthy Choices
Supported by a grant from Council on Aging of Silicon Valley and the Health Trust, the Womens Health Partnership (WHP) has received $20,000 to strengthen and make the Healthy Women, Healthy Choices curriculum more relevant to our priority population, medically underserved women. This award period will build upon assessment and revision activities initiated since May 2005. During this grant period, we hope to reach a minimum of 30 women who are community health center patients to participate in the community workshop series and 10 health/social service providers and community gatekeepers to participate in the Train-the-Trainer series. California Wellness Foundation funds the Partnership $250,000 for core operating support
The California Wellness Foundation approved $250,000 over the next three years to continue strengthening the infrastructure of the Partnership and to support the delivery of primary health care services at member clinics. Through this grant the Partnership will provide primary and preventive health care service to at least 325 low-income patients who do not have access to public or private health insurance. This grant will allow the Partnership to enhance and maintain its ability to provide administrative, information technology, and advocacy support to member CHCs by securing new offices, expanding fund development efforts, and equipping the new state-of-the-art Health Education and Training Center with telehealth capacity. Governors Budget Proposal 2007-2008: Health Care
Governor Arnold Schwarzenegger released his $143 billion Budget proposal this past January. The proposal plans to spend $103 billion in programs and services. Some programs such as Medi-Cal and Healthy Families will see increases while others will be slashed. The Governor plans to eliminate $5.5 billion from the states net operating deficit and at the same time he plans to have a reserve of $2.1 billion by the end of FY 08. When Proposition 57 was passed in 2004, the governor was allowed to sell $15 billion in bonds in order to reduce the General Funds deficit. Through this proposal he plans to pay $1.6 billion of the Economic Recovery Bonds that were sold in 2004. Governor Schwarzenegger is very optimistic about reducing the deficit but some fear that this proposal will raise serious policy and legal issues. Click here to read more. |

Career Expo 2007 at Andrew Hill High School
The Partnerships Healthy Futures Program coordinates an annual Health Career Expo in collaboration with Andrew Hill High Schools Medical Magnet Program. This school year, on February 27th, 2007 Healthy Futures Health Career Expo at Andrew Hill High School (AHHS) attracted approximately 450 students. Fourteen universities as well as colleges and community partners participated, bringing invaluable information to help open the pipeline to college and health careers for students. The Partnership was represented by Janet Espinoza and Veronica Villalobos, who led a fun, interactive game on what it means to be a Health Educator. Medical Peer Group students from AHHS worked hard to present research-based information and resources at their own tables on Diabetes, Teen Pregnancy, and Healthy Relationships. The event was successful and fulfilling for everyone. |

Community Health Partnership awarded Cancer Detection Programs: Every Woman Count (CDP:EWC) 3-year contract to serve Monterey, San Benito, Santa Clara, and Santa Cruz Counties
In late January, the Community Health Partnership was awarded a 3-year, $769,748 contract to oversee the California Department of Health Services Cancer Detection Section's (CDS) Cancer Detection Programs: Every Woman Counts (CDP:EWC) for Region 7 -- Monterey, San Benito, Santa Clara and Santa Cruz counties. Approximately $8.7 million will be awarded to 10 Partnerships throughout the state from March 1, 2007 through June 30, 2009. Click here to read more. |
