Gardner Expands South County Health Center

 

Gardner Family Health Network opened the doors to its expanded South County Health Center on October 16th. The new facility, which is over 10,000 square feet, is located in recently renovated downtown Gilroy. The state-of-the-art health center includes ten medical exam rooms, five dental operatories and a garden patio for patients and staff that will also be used for community events. Future expansion plans include 2,500additional square feet for primary care services.

 9th Annual Diabetes Breakfast 2006

 

On October 12th 2006, The Community Diabetes Project of the Community Health Partnership was proud to celebrate its 9th Annual Diabetes Breakfast at the Jacinto Tony Siquig Northside Community Center in San Jose as part of the Second Bi-national Health Week events.   This year’s theme was, “Overcoming Barriers to Systems Change: A panel Discussion to diabetes Care.”   A panel of members of the Diabetes Community Care Coordination Project included; Sonia Alvarez, Program Director for the Community Diabetes Project, Janet Espinoza Community Health Advocate for the Diabetes Community Care Coordination Project, James Reggio, Diabetes Services Coordinator at Indian Health Center and Maria Guerrero, program participant.

Steps to a Healthier Santa Clara County

 

The Community Diabetes Project is proud to announce it has been selected for the third year in a row to receive funding from Steps for a Healthier Santa Clara to conduct community-based diabetes awareness and education activities.  STEPS is a federally-funded program led by the Santa Clara County Public Health Department, focusing on the areas of asthma, diabetes, obesity, poor nutrition, physical inactivity, and tobacco use. The Community Diabetes Project of the Community Health Partnership contract includes community-based outreach, awareness and health education around the risk, treatment and management of diabetes.

Lupe Arango has been promoted to Health Policy Analyst.

Lupe will focus on policy analysis, advocacy and coalition building targeting priority populations; monitoring and analyzing information from governmental agencies regarding issues of concern to clinics, safety net providers and special populations; maintaining OSHPD data; working with CPCA on statewide data initiatives.  Lupe will continue to work on policy events and be the primary contact for legislators.  Congratulations, Lupe! 

 

Welcome to Veronica Villalobos!

Community Diabetes Project has hired Veronica as an Outreach Promotora for the Diabetes Community Care Coordination Project (DCCCP).  Veronica’s association with the Community Diabetes Project started earlier this year as part of the group of Promotoras from Promotores Unidos who worked in our STEPS for a Healthy Santa Clara County project.  She is bilingual and bi-cultural and very committed to serving the community.

8th Annual Jane Gremmell Cancer Symposium

 

On Thursday, October 19, 2006, nearly 70 health advocates, health care professionals, and cancer survivors attended our 8th Annual Jane Gremmell Cancer Symposium titled “Cancer Survivorship and Innovative Health Communication Tools.” Co-sponsored by American Cancer Society, this intimate, informative, and purposeful luncheon took place at David’s Banquet Hall in Santa Clara, California. Last year, this event was named in honor of Jane Marie Gremmell, a health advocate for breast cancer and other women’s health issues here in Santa Clara County.

Women’s Health Partnership Receives Mini-Grant To Reach Medically Underserved Asian Pacific Islanders

 

Asian American Network for Cancer Awareness Research and Training (AANCART) has awarded the Women’s Health Partnership (WHP)a $1000 mini-grant to educate medically underserved Asian Pacific Islander (API) women in Santa Clara County on breast cancer prevention utilizing WHP’s Healthy Women, Healthy Choices curriculum.   The primary goal of the mini-grant is to increase health status, including breast cancer, of medically underserved Asian Pacific Islander women by providing comprehensive health education and promoting adoption of healthier behaviors.

Community Health Partnership Collaborates with Stanford University School of Medicine to Integrate Population Health Into University’s Core Curriculum

 

The Partnership has had a long standing relationship with Stanford University’s School of Medicine’s Office of Community Health to assist with training their future physicians to effectively address today’s societal health needs, specifically in the area of policy/advocacy,  community health promotion, and community organizing.  Stanford University has subcontracted with the Partnership for $2500 during this next academic year to assist with shaping overall curriculum’s goal and objectives as well as develop module on physician advocacy.
