Member Community Health Centers 
Community health centers are one of the few sources of accessible, affordable, and culturally competent health care services for low-income and medically underserved populations. While maintaining a workforce that reflects the diversity of the communities they serve, these health centers provide an array of services including general family practice, medical and dental care, well baby visits, prenatal care, nutritional counseling, health education, and prevention, counseling, mental health, and community outreach.
Santa Clara and San Mateo Counties’ nonprofit community health centers have a combined budget of more than $30 million and provide health care to over 200,000 residents with more than 480,000 visits annually.

Indian Health Center of Santa Clara CountyThe Indian Health Center of Santa Clara Valley is a 501(c)3, nonprofit, community health clinic, located in San Jose, California. They serve people from all walks of life, regardless of race, creed, color, religion, sex, sexual orientation or disability and take pride in serving our diverse patients and clients. Besides having a highly talented medical and dental department, the Indian Health Center also has an outstanding counseling department, community wellness center and two WIC (Women Infants and Children) offices, one in San Jose and the other in Mountain View. |

With over 38 years of experience, NEMS is one of the largest health centers serving Asians in the United States. Our mission is to provide affordable, comprehensive, compassionate and quality health care services in a linguistically competent and culturally sensitive manner to improve the health and well-being of our community, regardless of ability to pay. Our staff speaks a variety of Chinese dialects and Asian languages, including Cantonese, Mandarin, and Vietnamese. |

Ravenswood Family Health CenterWe are a non-profit community health center that provides primary medical care and prevention services for all ages, including the uninsured and new immigrants, regardless of ability to pay. Our patients live in East Palo Alto, the Belle Haven community in Menlo Park, and the North Fair Oaks section of Redwood City. We have a staff of 85 full and part-time employees and contractors, including 24 medical professionals. |

RotaCare Bay Area, Inc.RotaCare Bay Area is a volunteer alliance of medical professionals, organizations and community members dedicated to providing free primary, quality healthcare services to uninsured families and individuals with limited ability to pay for medical care. We are entirely volunteer driven and supported solely through locally based philanthropy. |

San Jose Foothill Health CenterSan Jose Foothill Family Community Clinic is a vital resource for hundreds of thousands of uninsured and low-income women, men, and children in East San Jose who cannot afford insurance or private health care. Our clinic mobilizes physicians and additional health care providers in order to create an extensive health care delivery system serving the entire East San Jose. |

School Health Clinics of Santa Clara CountySince 1986, School Health Clinics of Santa Clara County has been providing easily accessible, quality health care to infants, children and adolescents on school campuses. Our clinics are located at schools in four school districts in east San Jose and Gilroy. They are open to all children in Santa Clara County. |
