Community Health Partnership Member Organizations

Community health centers and clinics are one of the few sources of accessible, affordable, and culturally competent health care services for low-income and medically underserved populations. While maintaining a workforce that reflects the diversity of the communities they serve, these health centers and clinics provide an array of services including general family practice, medical and dental care, well baby visits, prenatal care, nutritional counseling, health education, and prevention, counseling, mental health services, and community outreach.

Santa Clara and San Mateo Counties’ nonprofit community health centers have a combined budget of more than $30 million and provide health care to over 200,000 residents with more than 480,000 visits annually.

Our service area includes East Palo Alto, Belle Haven in Menlo Park, North Fair Oaks in Redwood City. Over one third of the residents were born outside the U.S; the majority come from Mexico, Central America and the Pacific Islands.

School Health Clinics of Santa Clara County

Since 1986, School Health Clinics of Santa Clara County has been providing easily accessible, quality health care to infants, children and adolescents on school campuses. Our clinics are located at schools in four school districts in east San Jose and Gilroy. They are open to all children in Santa Clara County.
