Health Home Transformation Initiative

 

The Health Home Transformation Initiative is an ongoing quality improvement project of the Community Health Partnership whose aim is to support our member community health center organizations achieve Patient Centered Health Home (PCHH) transformation. The Health Home Transformation Initiative follows the Institute for Healthcare Improvement (IHI) Breakthrough Series method for achieving large transformational change through collaborative learning. Our shared learning series, called the Learning Collaborative, is an approach that focuses on spreading and adapting best practices across multiple settings and creating changes within an organization that will promote improved quality of care, improved patient outcomes, and reduced costs. The Initiative is funded in part by Kaiser Permanente and Valley Health Plan, and will launch in January 2013.
