It is my pleasure to welcome you to The Community Health Partnership’s new website!
The Community Health Partnership is one of seventeen membership organizations throughout the state of California that represent non-profit Community Health Centers. We represent eleven primary care community‐based organizations providing health services at 31 sites, including five school health centers, three free clinics, an Indian Health Center and seven family planning clinics. The clinics combined serve 144,659 patients per year with 417,051 patient visits annually in Santa Clara and San Mateo Counties. Read more.
