Project History
In 1996, the Hospital Council Santa Clara Section began to fund and implement a community benefits project focusing on Latinos living with diabetes. The emphasis was on developing and improving access to education and primary care services. This effort gave birth to the Community Diabetes Project, a program of Community Health Partnership.
Mission and Objectives
The Community Health Partnership's Diabetes Project exists to reduce the incidence of complications of diabetes in Santa Clara County with a particular emphasis on the Latino Population. The primary objectives of the Community Diabetes Project are:
- To increase access to quality and accessible healthcare services for uninsured/underinsured diabetics in Santa Clara County with an emphasis on Latino men and women.
- To advocate for policy change that will increase access to healthcare and self management support including medical supplies for the intended population.
- To coordinate culturally and linguistically competent health education and chronic disease self-management training.
- To collaborate with medical providers, community based organizations and health-oriented agencies to maximize the use of resources and to coordinate efforts.
