Community Health Partnership is a consortium organization composed of community health centers. We believe in affordable, accessible, and quality patient-centered health care for all of Santa Clara and San Mateo Counties’ residents.
Mission
The mission of Community Health Partnership is to advocate for affordable and accessible health services for our diverse and multicultural communities regardless of socioeconomic, ethnic, religious, or cultural background.
Advocating for health care services by representing the Community Health Centers of Santa Clara and San Mateo Counties
The Community Health Partnership is one of seventeen membership organizations throughout the state of California that represent non-profit Community Health Centers. We are comprised of eleven primary care community‐based organizations providing health services at 31 sites, including five school health centers, three free clinics, an Indian Health Center and seven family planning clinics. The clinics combined serve 144,659 patients per year with 417,051 patient visits annually.
Disseminating Health Policies, Best Practices, and Coordinating Provider Education
At a time throughout our nation when the right to health care is being questioned or denied, it will be the Partnership’s role to provide education and analysis of such policies. Additionally, through our strong partnerships with Stanford University, San Jose State University and UCLA, we are involved in state-of-the-art community research and provider education in the areas of Breast and Cervical Cancer Detection, and HIV.
Being actively involved in Health Reform
The next several years will prove to be an exciting and fruitful time for community health centers. Together with our county partners, Community Health Partnership will help shape the implementation of federal, state and local initiatives that will improve access to health care to our most needy by:

Decreasing barriers

Streamlining referral systems,

Creating a culture of mutual responsibility and ownership of the patient by both community-based and county systems of care.
Serving as a source of Community referral to Health Care
Community Health Partnership will continue to serve as a source of referral to health care to the community-at-large. Our multi-lingual and multi-disciplinary staff is available to provide health information and guidance to callers and workshop attendees.
History
Community Health Partnership, Inc. (also known as “The Partnership”) was founded in 1993 in response to community concern for the viability of community-based, primary care clinics. By joining together in a local consortium, community health centers were able to enhance their services to low-income patients through the Partnership’s collaborative projects. Initially serving Santa Clara County clinics, the Partnership expanded into San Mateo County in 2004.
It is widely recognized that community health centers serve as an essential “safety net” for the area’s most vulnerable residents. The Partnership’s mission is to expand access to health services for the medically underserved, encourage prevention and health promotion through service, education and advocacy. The Partnership members and staff are committed to developing affordable health services that meet the needs of our diverse population. The Partnership’s general membership is comprised of eleven primary care clinic organizations providing more than 371,000 patient encounters at 31 sites, as well as one governmental agency.
The Partnership’s general members include Asian Americans for Community Involvement, Foothill Health Center, Gardner Family Health Network, Indian Health Center of Santa Clara Valley, Mar Monte Community Clinic, MayView Community Health Centers, North East Medical Services, Planned Parenthood Mar Monte, Ravenswood Family Health Center, RotaCare South Bay, Inc., School Health Clinics of Santa Clara County, and ex-officio member Santa Clara Valley Health & Hospital System. IIn addition, Community Health Partnership is fortunate to have countless partners whose ongoing financial support helps to achieve our organization’s mission.
Many of our member community health centers were founded to provide primary care to medically underserved populations. Their commitment to this mission holds strong to this day. All of the clinics have multi-cultural and culturally competent staff. IIn addition to primary care services, most of our member health centers offer an array of programs, including dental care, mental health, support groups, family planning, case management, health education, nutritional care, outreach, and prenatal services. With their combined budgets of over $64 million, member clinics are an important economic force in the community employing hundreds of health care professionals and providing critical services to a widely diverse population of undeserved patients.
The Community Health Partnership has forged a public-private partnership unique among community clinic organizations in the State, drawing participation from a variety of organizations which have normally not worked together. The Partnership has a track record of effective collaboration to improve preventive health services for “at risk” populations and successful community outreach.
